Step 1 - Download and complete our one-page Customer Application form
All new customers will need to complete this Customer Application form. If you have a question about any of the information requested on the form, please contact our call centre for assistance.
Step 2 - Download and complete our Direct Debit Mandate form
Direct debits are the quickest and easiest way of making payments. They ensure that your account is never overdue, so your orders can be processed without delay.
You'll receive advance notice of invoices via email, giving you time to check the details and contact Sanofi Pasteur if you have any queries - everything else will be done for you.

Step 3 - Send your completed Customer Application and Direct Debit Mandate forms to Sanofi Pasteur
